Employer-paid health insurance premiums are excluded from an employee’s gross income even though employers can deduct these payments as business expenses.[1] This ESI exclusion is our tax code’s largest tax expenditure, reducing federal revenue by almost $3 trillion between 2019-2028.[2] The ESI exclusion, because it subsidizes the purchase of health insurance, has contributed to overconsumption of health-care services and rising health-care costs.
