  A reader recently suggested (we apologize, we seem to have lost that email) that we do a 50-state survey of where the various states stand on this subject – along the lines of the post we did in 2008 on informal interviews with treating physicians.We thought that was a good idea, although it took us more time than we had hoped to put this together.
